RESEARCH PRIVACY NOTICE
Optional Forma Research
This notice applies only if Forma offers you research enrolment and you actively choose to join. Research is separate from Forma and the beta, and never changes Free or Pro access.
Controller and eligibility
Forma Pose Ltd, company number 17392462, is incorporated in England and Wales and is the controller. Its registered office is 23 Albert Embankment, London, United Kingdom, SE1 7GG. The service contact remains 10 York Road, London SE1 7ND, United Kingdom. Participants must be UK residents aged 18 or older. Contact eric@formapose.com.
Your separate choices
You may choose to share pseudonymous derived posture measurements, quality signals, check-in context and generated-report improvement records created after enrolment. This choice contains no camera image, video, audio, name or email.
You may separately choose to share silent clips made only from frames Forma analyses, paired with the derived posture and quality data for those frames. A clip may contain your face, background or another person in view. Camera-media sharing cannot be enabled without the paired derived-data choice.
Purpose, basis and access
Research copies are used only for the described approved research or evaluation purpose, never for advertising, employment, insurance, facial recognition, diagnosis or sale. Processing relies on Article 6(1)(a) consent and Article 9(2)(a) explicit consent for health-related derived information. Access is limited to named authorised Forma personnel, researchers or processors acting under written confidentiality, security and deletion obligations for the approved protocol; a new recipient, study purpose, labelling or model-training use requires its own approval and participant information.
Storage and retention
Research copies are held in a protected store separate from ordinary product Health Data and remain for no more than 30 days unless a specific approved study notice states a shorter period. Product Health Data remains on your computer. Consent and withdrawal evidence may remain while the account exists where necessary to demonstrate your choice.
Withdrawal, outcomes and rights
Leaving Research stops new Research collection immediately and does not affect product access. Forma then deletes the identifiable or pseudonymous Research copies it controls. Withdrawal cannot reverse genuinely anonymised results, findings or improvements that no longer relate to an identifiable person. Forma owns the service, methods and improvements it develops; participation does not create a right to ownership or payment unless a study expressly says otherwise.
Settings shows the current choices and withdrawal control. You may also email Forma to access, correct, erase, restrict or receive applicable Research information, withdraw consent or complain. You may complain to the Information Commissioner’s Office.
